Fabian Martinez III

On or about December 19, 2004, at approximately 3:00 a.m., in the vicinity of 6834 West Higgins, he was involved in a motor vehicle accident and failed to remain at the scene of the accident in violation of 625 ILCS 5/11-402.

On or about December 19, 2004, at approximately 3:00 a.m., in
the vicinity of 6834 West Higgins, he was involved in a motor
vehicle accident resulting in damage to a vehicle and failed to
give information and render aid in violation of 625 ILCS 5/11-
403.

On or about December 19, 2004, at approximately 3:00 a.m., in the vicinity of 6834 West Higgins, he was the driver of a vehicle involved in an accident resulting in injury to or death of any person, or in damage to property, including his own, in excess of $500, and failed to report the accident to the Administrator of the Illinois Safety and Family Financial
Responsibility Law within ten days in violation of 625 ILCS5/11-406.

On or about December 19, 2004, at approximately 3:00 a.m. in
the vicinity of 6834 West Higgins, he was the driver of a
vehicle involved in an accident and failed to give notice of the
accident by the fastest available means of communication to
the local police department in violation of 625 ILCS 5/11 -407.

On or about December 20, 2004, he committed disorderly
conduct when he knowingly transmitted or caused to be
transmitted in any manner to any peace officer, public officer or
public employee a report to the effect that an offense will be
committed, is being committed, or has been committed,
knowing at the time of such transmission that there is no
reasonable ground for believing that such an offense will be
committed, is being committed, or has been committed, in
violation of 720 ILCS 5/26-1.

On or about December 19, 2004, at approximately 3:00 a.m., in the vicinity of 6834 West Higgins, he was involved in a motor vehicle accident and failed to remain at the scene of the accident to give information and/or render aid and/or determine if aid was required.

On or about December 19, 2004, at approximately 3:00 a.m.,
in the vicinity of 6834 West Higgins, he was the driver of a
vehicle involved in an accident resulting in injury to or death
of any person, or in damage to property, including his own, in
excess of $500, and failed to report the accident to the
Administrator of the Illinois Safety and Family Financial
Responsibility Law within ten days.

On or about December 19, 2004, at approximately 3:00 a.m. in the
vicinity of 6834 West Higgins, he was the driver of a vehicle
involved in an accident and failed to give notice of the accident to
the local police department

On or about December 20, 2004, at approximately 4:50 p.m., he
falsely reported to the Chicago Police Department that his vehicle was stolen.

On or about April 6, 2005, he refused to cooperate in an
investigation being conducted by personnel from the Major Accident
Investigations Unit.

On or about April 6, 2005, he refused to cooperate with Lieutenant Gerard Carroll in an investigation being conducted by the Major Accident Investigations Unit.

On or about December 20, 2004, at 4:50 p.m., he falsely reported to the Chicago Police Department that his vehicle was stolen.
