On or about July 3, 2014, at about 0427 hours, at or around the intersection of 1111'1 Street and Longwood Drive, in Chicago, while on duty, following a collision between your Department vehicle and a civilian vehicle, you made a false report or reports, including to Sergeant Stephen Pietrzak and/or Police Officer Maria Hernandez, documented under Illinois Traffic Crash Report HX329110, to the effect that you had a green light at the time of the collision and the civilian vehicle had a red light, when in fact you had a red light and the civilian vehicle had a green light; thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.
