Maurice Willis

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, was in possession of a firearm while intoxicated and/or when there was a reasonable likelihood that he would be consuming alcoholic beverages, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, was in possession of a firearm which was not properly secured, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department. On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, was in possession of a firearm which was not properly secured, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, unnecessarily displayed and/or discharged his firearm, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

On or about, 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, failed to carry a police star or proper identification and/or certification while armed, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75t1 Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, carried a firearm loaded with ammunition not of one manufacturer, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

That from about 18 April 2000, to the present, or some period of time therein, Police Officer Maurice Willis, failed to cooperate with the Office of Professional Standards during the course of a C.R. investigation, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, did disrespect and maltreat any person, in that he engaged in a physical altercation with Darryl Davis.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, disobeyed General Order 92-3-3, II-D by failing to possess an approved Handgun Certificate while armed with a semi-automatic pistol.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, disobeyed General Order 92-03-01, IV-B-4 by carrying a firearm loaded with ammunition not of one manufacturer. On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75 th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, disobeyed General Order 92-03-01, IV-B-4 by carrying a firearm loaded with ammunition not of one manufacturer.

That from about 18 April 2000, to the present, or some period of time therein, Police Officer Maurice Willis, disobeyed General Order 93-3-2A, II-A-2, and/or General Order 93-03-02B, II-A-2 (issued 23 May 2001), by failing to cooperate with the Office of Professional Standards during the course of a C.R. investigation.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75 th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, did disrespect and maltreat any person, in that he engaged in a physical altercation with Darryl Davis.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, 2000, engaged in an unjustified physical altercation with Darryl Davis. On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75 th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, 2000, engaged in an unjustified physical altercation with Darryl Davis.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, was in possession of a firearm which was not properly secured, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

That from about 18 April 2000, to the present, or some period of time therein, Police Officer Maurice Willis, disobeyed General Order 93-3-2A, II-A-2, and/or General Order 93-03-02B, II-A-2 (issued 23 May 2001), by failing to cooperate with the Office of Professional Standards during the course of a C.R. investigation.

From about 22 March 2000 to the present, or for some period of time therein, Police Officer Maurice Willis failed to provide the Department with a current address and phone number.

On or about 21 October 1999, at approximately 0020 hours, while in the vicinity of 504 East 75th Street, Chicago, Illinois, Police Officer Maurice Willis, while off duty, unlawfully and unnecessarily used and/or displayed a weapon.
